Dedication Information

Dedications must be submitted at least two weeks prior to the Sunday requested. Note that once you submit this form your donation for the dedication can either be made with a check mailed to the church office or a payment online via Realm choosing 'Flower Guild Donation' as the fund. Please do not place the donations in the offering plate.

Preferred Date of Dedication

Dedications provided less than two calendar weeks before a preferred date will not be able to be fulfilled. Also, if there is a conflict with your preferred date, a member of the Flower Guild will contact you directly to discuss options.
