Dedication Information

Dedications must be submitted at least one week prior to the Sunday requested. Note that once you submit this form your donation for the dedication can either be made in the offering plate with an envelope noted towards your dedication, a check mailed to the church office, or a payment online via Realm choosing 'Flower Guild Donation' as the fund.

Preferred Date of Dedication

Dedications provided less than a calendar week before a preferred date will not be able to be fulfilled. Also, if there is a date conflict with your preferred date a member of the flower guild will contact you directly to discuss options.
